Community Engagement Specialist - Job Summary
Lead the Connect for Life program by engaging community members through events, outreach, and personalized support to promote health and build connected communities.
Key Responsibilities
- Lead the Connect For Life program using Wider Circle's curriculum to build trust and promote health.
- Plan and host virtual and in-person events to foster member relationships and wellness.
- Proactively contact members to ensure participation through calls, home visits, and group activities.
- Document all member interactions and attendance using Wider Circle’s digital systems.
- Identify community leaders and invite them to become Wider Circle Ambassadors.
- Outreach to prospective members and develop personalized enrollment plans.
- Serve as liaison connecting members to health plans and healthcare services.
- Achieve membership engagement and growth goals monitoring metrics like attendance and feedback.
- Manage and report hours worked remotely.
- Conduct in-home visits for members unreachable by other means.
Requirements
- Bilingual in English and Spanish required.
- Strong team player with ability to provide leadership.
- Proactive and adaptable in a fast-paced environment.
- Preferred Pennsylvania Community Health Worker Certificate.
- 3+ years experience in community outreach, teaching, or facilitation.
- Experience with community building for groups of 500+ members.
- Organizational skills with ability to prioritize and follow through.
- Comfortable using web- and app-based systems.
- Able to work independently with minimal supervision remotely.
- Reliable transportation and willingness to travel 20-50 miles for home visits.
- Flexible schedule including outside regular business hours.
- Willingness to complete drug testing and background checks.
Education
Associate's degree preferred; 5+ years relevant experience may substitute.
