Community Engagement Lead - Job Summary
The Community Engagement Lead is responsible for building trust and relationships with members, engaging them through various outreach methods to improve health outcomes and support community health and wellness initiatives.
Key Responsibilities
- Build relationships with individuals and communities to promote health and wellness.
- Conduct community-based outreach and education on healthcare services, disease prevention, and healthy lifestyles.
- Advocate for member and community needs within healthcare systems and social services.
- Establish partnerships with community-based organizations to broaden support services.
- Provide culturally appropriate health education on nutrition, chronic disease management, mental health, and preventative care.
- Support individuals in understanding and navigating the healthcare system.
- Organize workshops and informational sessions on healthcare topics.
- Assist individuals in accessing medical care, insurance programs, and social services.
- Help members close care gaps by scheduling appointments, reminders, assisting with paperwork, and explaining treatment plans.
- Identify barriers to healthcare (Social Determinants of Health) and provide resources.
- Connect individuals with social services such as housing, food assistance, and employment support.
- Document interactions in digital systems in a timely manner.
- Support Area Manager by assisting and training Specialists as needed.
- Support membership standards with minimal supervision.
- Conduct in-home visits when necessary to review member needs and access to care.
- Drive safely to public places up to 30 miles from home, carry supplies, and effectively communicate in-person and virtually.
Requirements
- Strong passion for helping and serving others.
- Outgoing personality with a positive attitude to engage in-person and via phone or video.
- Genuine interest in working with Medicare/DSNP/Medicaid populations and commitment to supportive relationships.
- Strong verbal and written communication skills for diverse community engagement.
- Proactive outreach approach and ability to lead community engagement initiatives.
- Adaptability to dynamic work environments and willingness to learn and grow.
- Experience working remotely and ability to work autonomously.
- Flexible availability outside business hours and reliable transportation.
- 3-5 years of relevant experience.
Education
Associate or Bachelor Degree preferred
