Community Engagement Associate - Job Summary
Lead and manage the Connect for Life program in Detroit, engaging members through events, one-on-one outreach, and building a supportive community to promote health and wellness.
Key Responsibilities
- Lead the Connect For Life program using Wider Circle's curriculum to build trust and promote health.
- Plan and host virtual and in-person events to foster member relationships and wellness.
- Proactively contact members to ensure participation in events and activities, including home visits.
- Document all member interactions, outreach, and attendance using online systems.
- Identify and recruit community leaders as Wider Circle Ambassadors to enhance member engagement.
- Reach out to prospective members and develop plans to enroll them in the program.
- Serve as liaison connecting members to health plans and assist with healthcare services and appointments.
- Achieve membership engagement and growth goals through monitoring attendance and feedback.
- Conduct home canvassing to meet members unable to attend events and review their needs.
- Manage time and report hours worked in a remote environment.
Requirements
- Certified as a Community Health Worker in Pennsylvania.
- Ability to work as a team player and contribute positively with leadership when needed.
- Proactive approach and comfortable taking initiative with minimal direction.
- Adaptable to fast-paced and changing environments with eagerness to learn.
- 3+ years experience in community outreach, teaching, or facilitation; healthcare experience is a plus.
- Experience creating or supporting a community of 500+ members.
- Strong organizational and task prioritization skills.
- Comfortable with web- and app-based systems (tech-savvy).
- Ability to work independently in a remote environment with little supervision.
- Reliable transportation and willingness to travel 20-50 miles for home visits.
- Flexible schedule availability including outside regular business hours.
- Willingness to complete pre-employment drug testing and background checks.
- Passionate about helping others, excellent communication and presentation skills.
- Outgoing and able to engage people both in person and virtually.
- Strong networking and leadership skills with peer guidance experience.
Education
Associate's degree preferred; 5+ years relevant experience may be considered instead of degree
