Community Engagement Specialist - Job Summary
Lead the Connect for Life program to build trusted relationships among members, fostering engagement through events, outreach, and personalized support to promote better health outcomes.
Key Responsibilities
- Lead the Connect For Life program using Wider Circle's curriculum to build trust and promote health
- Plan and host virtual and in-person events to foster member relationships and wellness
- Proactively contact members to ensure participation in events and activities including home visits
- Document member interactions and outreach in digital systems timely
- Identify and recruit community leaders as Wider Circle Ambassadors
- Conduct outreach to prospective members and develop personalized enrollment plans
- Serve as liaison connecting members with health plans and healthcare services
- Achieve membership engagement and growth goals with accountability
- Manage time and report hours worked remotely
- Conduct home visits to meet members and encourage event attendance
Requirements
- Team player with leadership abilities
- Proactive and able to initiate community engagement with minimal direction
- Adaptable to fast-paced environments and eager to learn
- Pennsylvania Community Health Worker Certificate preferred
- 3+ years experience in community outreach, teaching, or facilitation
- Healthcare experience is a plus
- Experience supporting or creating communities of 500+ members
- Strong organizational and prioritization skills
- Comfortable using web and app-based systems
- Able to work independently in a remote environment
- Reliable transportation with willingness to travel 20-50 miles
- Flexible schedule including outside regular business hours
- Willing to complete pre-employment drug testing and background checks
- Passionate, communicative, outgoing, positive, and skilled in networking and leadership
Education
Associate's degree preferred; 5+ years relevant experience may substitute
