Community Engagement Lead - Job Summary
The Member Engagement Lead will build trusted relationships with members to improve health outcomes through various outreach methods and serve as a mentor to peers in the organization.
Key Responsibilities
- Build relationships with individuals and communities to promote health and wellness.
- Conduct community-based outreach and education on healthcare services and healthy lifestyles.
- Advocate for member and community needs within healthcare and social services.
- Establish partnerships with community organizations to enhance support services.
- Provide culturally appropriate health education on nutrition, disease management, mental health, and preventative care.
- Help individuals navigate healthcare systems and access medical care and social services.
- Organize workshops and informational sessions on healthcare topics.
- Support members in closing care gaps through scheduling, paperwork assistance, and treatment plan understanding.
- Identify healthcare barriers and connect individuals with social services.
- Document interactions in digital systems promptly.
- Support Area Manager by training and assisting Specialists as needed.
- Conduct in-home visits to meet members and discuss care access when necessary.
- Drive safely to public places up to 30 miles away and carry supplies up to 30 pounds.
- Effectively communicate in person, by phone, or virtually.
Requirements
- Strong passion for helping and serving others.
- Outgoing personality with positive attitude for in-person and virtual engagement.
- Interest in working with Medicare/DSNP/Medicaid populations and building supportive relationships.
- Strong verbal and written communication skills for diverse community members.
- Proactive in outreach and able to lead community engagement initiatives.
- Adaptable to dynamic work environment, eager to learn and grow.
- Experience working remotely and autonomously.
- Flexible availability including outside business hours and reliable transportation.
- 3-5 years of relevant experience.
Education
Associate or Bachelor Degree preferred
